We are pleased that you are considering purchasing a Right of Inurnment in the Columbarium of Preston Hollow Presbyterian Church, and appreciate the confidence you have invested in the Columbarium Committee. All application materials and instructions are included on this page. You may wish to print this page for easy reference. Downloadable materials are marked with a blue link. Please read over all materials carefully before sending us your application. In order to view the materials below you must have the Free Adobe Acrobat Reader installed on your computer. NOTE: A completed application should contain three things:
- The Application to Purchase a Right of Inurnment, filled out and signed. Please type or print clearly.
- The Individual Niche Inscription Form, filled out and signed. Please type or print clearly.
- Payment by check of $1,500 per niche, made out to "Preston Hollow Prebyterian Church," with the notation, "Columbarium." OR If you pay by credit card, please include the name as it appears on your credit card, your credit card number and date of expiration on your application.
To assist you in completing the application process, please reference the numbered list below.
- I have read over the Rules, Policies and Regulations Related to the Columbarium, found on this Web site - These are approved by the Session and are the authority for all decisions made in regard to the use of the Columbarium. When you purchase a Right of Inurnment you agree to abide by this document, so please read it carefully. You may print a copy of this document for your files from this website.
- I have found a wall and niche using this Web site or by visiting the Columbarium - You will need to indicate on your application the location you prefer for your niche(s). You can use the Interactive Niche Lookup Chart for guidance, or you may visit the Columbarium. If you choose to visit the Columbarium to find your wall and niche, you can use one of the folders found at the two entrances to the Memorial Garden to select your niche. After you have found a wall and niche, make note of it for your application.
- I have filled out the following forms:
- I have included payment of $1,500 per niche You may pay by check payable to "Preston Hollow Presbyterian Church," with the notation, "Columbarium." If you wish to pay by credit card, please include the name as it appears on your credit card, your credit card number and date of expiration on your application.
When you have completed steps 1-4 above, please mail the Application to Purchase a Right of Inurnment, Niche Inscription Form and check or credit card information to:

After verifying the information on your Application, the Columbarium Committee will return a photocopy of your complete application to you, along with a Certificate of Right of Inurnment.
If you have problems accessing or downloading any information on this web site, you may still obtain a Columbarium Packet from the literature rack off the Atrium, or by calling Alice Dohm in the church office at (214) 368-6348, ext 113.
A supply of the urns for cremains is on hand at the church. Remember that, due to the size of the Columbarium niches, the only urn that may be used is the urn furnished by the Columbarium, at no additional cost.
A sampling of funeral homes offering cremation services, with their prices, is included on this web site (see Funeral Home Arrangements). You are urged to complete your cremation arrangements and forms promptly, retaining one copy and having one copy placed in your file at the church.
